Room Assignment Policy for Groups
In an effort to ease confusion surrounding room assignments for individuals and groups requesting space at West Side United Methodist Church, The Board of Trustees have established guidelines for assigning of rooms.
The form is downloadable as a Microsoft Word document. To download the form right click here and select "Save target as" or "Save link as".
The form can then be opened in MS Word. It may then be completed in one of three ways:

Fill out the form using your computer, save it as a document, and email it as an attachment to Arlene at the church office. Send to: westside@westside-umc.org,or |
2.

Fill out the form using your computer, then print out and mail or drop it off at the church office, or |
3.
|
Print out a blank form, fill it out by hand, and drop it off or at the church office (or send by mail.) |
